Step 3 - Select  and print an Benefit Upgrade(s) (optional).

• Enhanced Plan Discount Card
• Enhanced Plan Discount Card with PPO Network

Insured Prescription Card

• Insured Prescription Card with $ 5 Co-pay
• Insured Prescription Card with $10 Co-pay
• Insured Prescription Card with $15 Co-pay

Step 4 - Select and print the Employee Enrollment Form for your group. 

Step 5 - Mail the Application, Enrollment forms and premium directly to Markel.
