Employer Groups

In most states where the plan is available, groups with 4 or more eligible employees may participate. Due to state regulations the plan may only be available to groups with 51 or more eligible employees in some states. Please check "Availability" to determine if the program can be offered to your employees.

Employees

Full-time, part-time, seasonal and temporary employees, and their families, can participate in the MBHI plan if they meet the following requirements:

Eligible Employees

All employees in an eligible class who have completed the required waiting period (30, 60, or 90 days, if elected by the employer) are eligible provided they: are actively at work performing all the normal duties of their job; reside in the United States; are not in full-time service of the Armed Forces.

Eligible Dependents

Employees’ dependents are also eligible. Spouses (if not legally separated or divorced) and children, including adopted and stepchildren who are unmarried and dependent on the employee for support, up to age 19 (26 if a full-time student), are eligible provided they: are performing the normal duties of persons of like age and gender; reside in the United States; are not in full-time military service.
